Effective March 31, 2017, I will be leaving the Department of Orthopedic Surgery at Columbia University Medical Center and joining the faculty at NYU Langone Medical Center. I value you as a patient, and am committed to making sure you continue to receive the best orthopedic spine care. If you would like to continue to see me at my new position, please visit my website for the most up to date information, at www.drfischerspinesurgeon.com. If you would like to continue your care with ColumbiaDoctors, I have arranged to transition your care to one of our other highly qualified spine surgeons in the Department of Orthopedic Surgery here at ColumbiaDoctors and The Spine Hospital at New York-Presbyterian/Allen. You can rest assured that the team at The Spine Hospital will be able to provide you with expert ongoing care. They have offices in New York City, Westchester, and Englewood, New Jersey.

 

In the event you should wish to continue your care with me, or transfer your care to another physician who is not part of ColumbiaDoctors, we can assist you in transferring your medical records. You will need to complete and sign a medical release form. This form can be obtained by calling our Medical Records Department at: (212)305-0099, requesting by E-mail via: medrecrequest-ortho@columbia.edu, or by following the link: https://columbiaortho.org/patients/medical-record-information. Once our department has received the signed form, we will forward your medical records to either my new office or another spine specialist outside of ColumbiaDoctors.
